A CORONER has heard how a woman detained under the Mental Health Act at the Whiteleaf Centre in Aylesbury, Bucks, managed to take their own life despite being under observation.
Amanda Gibbens was taken to Stoke Mandeville Hospital in July 2020 after the incident on the Ruby Ward but died. The inquest held last month, concluded the cause of death was Hypoxic Brain Injury as a result of Asphyxiation.
The Whiteleaf Centre is managed by Oxford Health NHS Foundation Trust.
Whilst at the centre as an inpatient she was placed in a de-escalation room. During a visit to the bathroom Ms Gibbens made a further attempt to harm themselves which resulted in a cardiac arrest.
Despite resuscitation efforts she did not survive.
